Connections online that billing shows as suspended. Accounts online after their expiry date. Logins that billing has never heard of. Speeds above the plan. One login in use from two places at once.
Why it happens
Usually nobody did anything wrong. A manual unblock was never reversed, or a plan changed in one system and not the other.

Export your online sessions and your active accounts, and compare the two lists. Fix what you find, and repeat it every month.
